The OpenSource Version of xforms has moved from the xforms website 
(http://world.std.com/~xforms) to a new site 
(http://www.nongnu.org/xforms/), but there is  no source tarball available 
yet.  You can get a cvs snapshot by following the instructions at 
(http://savannah.nongnu.org/cvs/?group=xforms) 

I have the 1.0 release from some time ago, and that allowed me to compile 
LyX 1.3.2 ok.
