[ydl-gen] YDL 6.0 on PM G5: can't boot! help!?!

Reformat the ultra0 disk with Mac OS X ensuring it uses an Apple
partition table and then reinstall YDL6.
The disk probably had a MSDOS partition table on it, and instead of
reinitializing the disk, it simply added partitions.
