bootstrap disappears

For the second time now, my bootstrap partition has disappeared.  When I
boot witht he option key held down, I only see a MacOS partition to boot
from. When I use Open Firmware with "setenv boot-device hd:10"  yadda yadda
yadda...  I get the disk with a question mark???  Why is this happening?  I
have not shut the machine down improperly, and this hard disk is new!

Anyone else have this happen?
