KFloppy is able to read and write, but not format??  Can you do any of these
operations from the command line?  Sounds like KFloppy might have a bug.

Try linking to fd0 instead of fd0H1440.
	ln -s /dev/sda dev/fd0

> # mkdosfs /dev/sda

Also, make sure that /dev/sda is not mount before you try this.
