Kernel version, kernel configuration information, and dmesg output
right after boot would probably be helpful.

For whatever it's worth, I've never had this problem on our Xserve and
YDL 2.3.  I always use a kernel I've compiled myself.  The disks (two)
are completely RAIDed with Linux software RAID (/boot and / are mirrored,
the rest is striped), so I don't get to explicitly copy files from one
disk to another (but of course, the mirroring does exactly that in the
background).
