username/password not accepted

Username/passwords are case sensitive.  Could this be it?

formatted with Drive Setup
single, 20gig drive, 1gig, 256meg, balance for ydl
running os9.2.1 on old world g3 tower with 448meg ram

after install, attempt to boot into ydl, username/password are invalid

I wiped everything, wrote username/password ahead of time, installed, 
and copied username/password into install. same problem.

fyi - i am a support tech for a data integration software company, i 
did the step-by-step on this and failed.

any tips?
