Hi Ben:

Thanks for your help...

I tried resetting the PRAM and defragmenting the disk - neither made any
difference. However, I replaced the "Mac OS ROM" file that I had been
using (v3.0) with v3.8 from the "Ethernet Update 1.0" from Apple, and MOL
started working!

So, at least in my case, the cause of the "file system map inconsistent"
error seems to have been using "Mac OS ROM" 3.0 instead of 3.8...no other
changes that I made to my enviroment had any effect.
